The Activate Wellness Program is for anyone who wants to optimize their health. If you have a condition which requires urgent medical attention, or if you are not ready to invest in your health, or if you are not motivated to take responsibility for your health through significant nutrition and lifestyle changes, the Activate Wellness Program is not a good fit for you.

Yes. We require for all our patients to have a Primary Care Physician. We do not provide acute urgent care services, and we do not replace the services that your current medical team provides. We provide a stand-alone service focused on eliminating root causes, and creating and advancing health.

Yes, we actively encourage additional family members to join. We offer a $500 credit for each family member that joins.

We strongly believe that third party payers should not interfere with the relationship between the patient and the doctor, influence medical decisions, or limit access to care and time we spend with our patients. Therefore we are a proud direct care provider.

We do not accept insurance or Medicare for our consulting services. Your case requires special attention, educational resources, communication, and time that is not covered by insurance companies.

We will provide you with an itemized bill with ICD-10 diagnostic codes and CPT codes that you can submit to your insurance provider for out-of-network benefit reimbursement for your appointments and acupuncture treatments according to your insurance coverage. Some of the laboratory testing we collect may be submitted through your insurance. We do not guarantee reimbursement. You may also use your pre-tax dollars from your Health Savings Account (HSA or FSA) account to pay for your consultations and lab fees. The functional lab tests we use are often covered by PPO insurance plans or can be purchased at a wholesale price.

Labs and supplements are not included in our Activate Wellness Program. We offer direct lab pricing so that you get the best prices on recommended lab test. The blood test will offer insurance coverage as well as some functional labs will offer insurance coverage for PPO plans as well as cash discounts. We offer VIP Supplement pricing through our extensive in-house and online pharmacy dispensary.

We are looking for people who are motivated, dedicated, and ready to do what it takes to optimize their health. We are not looking for individuals who simply want to complete testing and do not implement the necessary lifestyle changes. With that being said, if you are generally healthy and are looking to complete testing to remain in that state, we highly encourage it. In this case the Activate Wellness Program would be a good option to consider.

We are committed to enhancing your access to your services, laboratory tests, and medicines you need through transparent and affordable pricing. We provide labs and supplements at our cost – no markup – and pass the savings on to you. 

By becoming a partner you have access to direct pricing for labs, which ensures you invest the lowest price possible. Some labs we work with use coverage for PPO plans, as well as cash discounts. PPO Insurance Plans, HSA, and FSA benefits may be used to cover lab costs depending on your insurance provider.

On average, you should budget between $700 – $1800 for functional lab tests.

The Optimal Wellness Program is not typically covered by insurance, however in many cases you can use your HSA or FSA for your visits.

Blood work is done through your local Lab Corps. or Quest Diagnostics or in the comfort of your home with a local phlebotomist who will come to you.

Functional lab tests are typically done at home with a kit and clear instructions we provide you with. Many of the tests have shipping included in the cost of the kits. The investment is made prior to shipping out the test kits.

Yes, we want to make sure that you fully understand your lab results and that you can easily share them with your healthcare team if necessary. We spend time with you explaining in detail the results of your lab and discuss the proactive steps for you to take. We believe that an informed partner gets much better long-term outcomes and becomes an active participant in their health and wellness.

Many patients come to realize that a healthy mind, body and spirit is priceless and as such they find comfort and value in continuing ongoing support from Dr. Feldman and our team after completion of our Activate Wellness Program. Upon completion of the program you qualify for the Become Optimal graduate program which is highly encouraged to ensure the hard work you put into your health is maintained and continues to improve.  We provide this service on an al a carte basis. Average range of our Activate Wellness Program is from 6 – 18 months.

That depends on what you are taking, and how much progress you can make. Patients who consult with Dr. Feldman are often able to eliminate medications or significantly reduce the amount of medications they are on. With that being said, we do not monitor drug therapy. If you are to discontinue your medication that will be by your prescribing physician, not by Dr. Feldman.

Yes, you will have an option to email us if you have any questions/concerns. We do ask that if the response is longer than a couple paragraphs that you call the office to speak with a support team to address them. You can expect a response within 48 hours.

If we are a good fit for each other, the next step is to schedule a Comprehensive Functional Medicine Health Assessment. You will be required to put a non-refundable deposit of $250 down which will go towards the cost of your program.
