Comprehensive New Patient Package
Your Comprehensive New Patient Package Includes:
- Prior to your appointment, you will be required the in-depth created intake forms to help us understand your health.
- Review of your past medical records: Dr. Alice will review all past medical records or laboratory results that you provide. Often, there are important insights into your health within those records such as patterns or dots that haven’t been previously connected.
- Two Functional Medicine Health Assessment with Dr. Alice:
- ONE 75-Minute Comprehensive Functional Medicine Health Assessment:
- Discuss & Review your health history, current symptoms and primary health concerns.
- Review your current lifestyle, including: eating behaviors, nutrition, activity levels, stress levels, and sleep habits.
- Discuss your health & wellness goals.
- Go over your Metabolic Assessment & Brain Function forms.
- Use ART Kinesiology for functional assessment of your core biological systems.
- Nutritional Physical exam as needed.
- Functional Medicine assessment.
- With a clearer picture of your history, timeline and functional assessment, we will recommend Functional Medicine lab tests to uncover overlooked causes and assess your health on a cellular level. You’ll leave this appointment with recommendations for initial functional lab tests specifically tailored to your case, as well as some preliminary treatment recommendations to start implementing. However, further customization for your plan will be provided at your Case Review appointment, after lab results have been received and there has been time to thoughtfully consider all aspects of your case.
- ONE 75-Minute Case Review and Action Plan Appointment: This visit is scheduled about 3 – 6 weeks after the initial Functional Medicine Health Assessment. At this visit you can expect a thorough explanation of your laboratory and Functional Medicine test results, and a discussion of Dr. Alice’s research and findings regarding your condition. You’ll have plenty of time to ask questions. We’ll spend time talking about your best plan for reaching your goals.
- ONE 75-Minute Comprehensive Functional Medicine Health Assessment:
Understanding Your Answers include:
- Functional Medicine Testing: Part of your plan may require specialized Functional Medicine testing such as: analysis of your gut microbiome, testing gut infections and parasites, Candida, hormone and adrenal gland testing, food sensitivity testing, heavy metal testing. You will access to these tests and receive special discounts and direct lab pricing. The package price does not include the cost of these tests.
- Advanced Blood Chemistry Analysis: To get the most out of your blood chemistry results, we use advanced training and technology to get important nutrition and health status data from your blood test.
- Detailed lab interpretations (stool, saliva, urine): We will discuss any underlying imbalances or impediments to optimal health revealed by these tests.
- Your Personalized Healthy Eating Plan: Food is the most powerful medicine. Together, we’ll discover the best eating plan for your long-term success. You’ll receive as much support as needed, from basic guidelines to check-lists, meal plans or shopping lists. You’ll learn tricks and tools to make food your ally and you’ll still be able to indulge in foods you enjoy. After thoroughly evaluating your case, we create your personalized plan. It is important that we do the right things in the right order, to help you achieve the best results.
- Customized supplement protocols for each phase of your healing: You will receive personalized treatment plan at each phase of your healing journey. Treatment phases are typically 30 – 45 days long, and consist of specific nutrition, lifestyle and supplement protocols for that phase of healing. Then we will move on to the next phase with a new plan.
- Customized Strategic Action Plans: After each consultation with Dr. Alice, you will walk away with a clearly written action plan that specifically addresses your health challenges. Your plan will include specific diet and lifestyle recommendations, along with specifically targeted supplements.

- You’ll have answers to your most pressing questions and you’ll begin to address the challenges and implement the lifestyle changes that will help you to overcome the obstacles that have been keeping you stuck.

Here’s How it Works:
- After we’ve talked in the 20-Minute Discovery Call, you’ll decide that you’d like to move forward and become a new patient.
- You’ll then be given access to the Comprehensive New Patient Intake Forms and given instructions on how to obtain records from your other healthcare providers.
- After the New Patient Intake Forms and records have been received, you’ll be invited to schedule your New Patient Appointment. You’ll also receive a link to pay for the Comprehensive New Patient Package which will secure your 75-minute Functional Medicine Health Assessment as well as your 75-minute Case Review and Action Plan.

- 2. When you are ready to book your 20-Minute Discovery Call, click the link below to fill out the Discovery Call Application form.
- 3. After submitting the form, it will take 48-72 hours for our patient coordinator to reach out and schedule your Discovery Call.
- 4. After you’ve booked, you’ll receive a confirmation email with the call-in details.
- 5. At your scheduled time, please give my office a call and we’ll chat.
- 6. At the end of the call, if we choose to work together, my next available patient visit is yours.
