How long should I expect the call/consultation to last?

Expect the call/consultation to last 20 minutes. And if you are a good candidate for our care, we will go over the paperwork to get you started. This will be an additional 5 minutes, we value your time, and do our best to provide you with a timely service.

At your initial new patient evaluation, Dr. Feldman will listen to your story, review your medical history, Health & Metabolic Assessment Forms, perform a physical exam and Functional Medicine Assessment Evaluation. To get the most value from your initial evaluation, complete and return your intake form 24 hours in advance. Bring any pertinent medical records, test results, list of medications, and current supplements.

How do I know if I qualify for the Optimal Wellness Health Transformation Program?

The Optimal Wellness Health Transformation Program is for anyone who wants to optimize their health. If you have a condition which requires urgent medical attention, or if you are not motivated to take responsibility for your health through significant nutrition and lifestyle changes, the Optimal Wellness Health Transformation Program is not a good fit for you.

Do I still need a Primary Care Physician?

Yes. We require for all our patients to have a Primary Care Physician. We do not provide acute urgent care services, and we do not replace the services that your current medical team provides. We provide a stand-alone service focused on eliminating Root Causes, and creating and advancing health.

Does the cost of the new patient evaluation apply to the Optimal Wellness Health Transformation Program?

Yes, your new patient evaluation is one of your Comprehensive Appointments included in your Optimal Wellness Health Transformation Program. When you enroll in the Optimal Wellness Program, your payment for the initial new patient evaluation is credited to the total investment of the program.

Our Optimal Wellness Health Transformation Program starts at $2,497 In addition there are flexible payment plans, by making a $499 deposit at the time of booking for the new patient evaluation and choosing one of the following options to complete payment:

We accept cash, checks, major credit/debit cards. We also offer a credit option with 0% financing.

What if I am not a financial decision maker of my Health Care needs?

If you are not the financial decision maker of your Health Care needs, please make sure to bring your significant other with you for your initial comprehensive new patient evaluation appointment.

What if my spouse/kids want to join me – do you offer family discounts?

Yes, we actively encourage additional family members to join. We offer a $500 credit for each family member that joins.

Does your office take insurance for Root Cause & Functional Medicine Services?

We strongly believe that third party payers should not interfere with the relationship between the patient and the doctor, influence medical decisions, or limit access to care and time we spend with our patients. Therefore we are a proud direct care provider.

We do not accept insurance or Medicare for our consulting services. Your case requires special attention, educational resources, communication, and time that is not covered by insurance companies.

We will provide you with an itemized bill with ICD-10 diagnostic codes and CPT codes that you can submit to your insurance provider for out-of-network benefit reimbursement. Some of the laboratory testing we collect may be submitted through your insurance, but we cannot guarantee they will cover any or all of the costs. You may also use your HSA or FSA account to pay for your consultations and lab fees.

Can I use my HSA or FSA account?

Yes. Please check with your HR department to be sure.

Are labs and supplements included?

Labs and supplements are not included in our Optimal Wellness Health Transformation Program, and are additional cost, depending on what you need.

We offer VIP Supplement pricing through our extensive in-house pharmacy.

Can I just get testing done?

We are looking for people who are motivated, dedicated, and ready to do what it takes to optimize their health. We are not looking for individuals who simply want to complete testing and do not implement the necessary lifestyle changes. With that being said, if you are generally healthy and are looking to complete testing to remain in that state, we highly encourage it. In this case the Optimal Wellness Health Transformation Program would be a good option to consider.

How much should I budget for the Functional Lab Tests?

We are committed to enhancing your access to your services, laboratory tests, and medicines you need through transparent and affordable pricing. We provide labs and supplements at our cost – no markup – and pass the savings on to you.

By becoming a partner you have access to direct pricing for labs, which ensures you invest the lowest price possible. Some labs we work with use coverage for PPO plans, as well as cash discounts. PPO Insurance Plans, HSA, and FSA benefits may be used to cover lab costs depending on your insurance provider.

On average, you should budget between $700 – $1800 for functional lab tests.

Is the Optimal Wellness Health Transformation Program and Functional Lab Testing covered by insurance?

The Optimal Wellness Program is not typically covered by insurance, however in many cases you can use your HSA or FSA for your visits.

Upon request we will provide an insurance form called a Superbill for your appointments that you can submit to your insurance provider to receive out-of-network benefit reimbursement if that is part of your plan. We do not guarantee reimbursement. The functional lab tests we use are often covered by PPO insurance plans or can be purchased at a wholesale price.

How does the lab testing work?

Blood work is done through your local Lab Corps. or Quest Diagnostics or in the comfort of your home with a local phlebotomist who will come to you.

Functional lab tests are typically done at home with a kit and clear instructions we provide you with. Many of the tests have shipping included in the cost of the kits. The investment is made prior to shipping out the test kits.

My doctor never shares my lab results with me – will I have access to them?

Yes, we want to make sure that you fully understand your lab results and that you can easily share them with your healthcare team if necessary. We spend time with you explaining in detail the results of your lab and discuss the proactive steps for you to take. We believe that an informed partner gets much better long-term outcomes and becomes an active participant in their health and wellness.

What happens after I complete the Optimal Wellness Health Transformation Program?

Will I have to stop taking my current medications?

That depends on what you are taking, and how much progress you can make. Patients who consult with Dr. Feldman are often able to eliminate medications or significantly reduce the amount of medications they are on. With that being said, we do not monitor drug therapy. If you are to discontinue your medication that will be by your prescribing physician, not by Dr. Feldman.

Can I email the Optimal Wellness Team my questions?

Yes, you will have an option to email us if you have any questions/concerns. We do ask that if the response is longer than a couple paragraphs that you schedule an appointment.
